Reporting Incorrect Facility Information

FindHosi strives to provide accurate information about healthcare facilities. However, facility details may sometimes be outdated or incorrect. This guide explains how to report inaccurate information to help us improve the service for everyone.

Types of Information You Can Report

You can report various types of incorrect or outdated information about healthcare facilities:

  • Incorrect contact information (phone number, email, website)
  • Wrong address or location on the map
  • Outdated operating hours
  • Inaccurate services or specialties listed
  • Facility closure or relocation
  • Incorrect facility type (public vs. private)
  • Missing or incorrect emergency services information
  • Facility name changes or rebranding

How to Report Incorrect Information

There are several ways to report inaccurate facility information in FindHosi:

Method 1: From Facility Details

  1. Open the facility details by clicking on a facility in Map View or List View
  2. Scroll to the bottom of the facility details
  3. Click the "Report Incorrect Information" link
  4. A reporting form will open where you can specify what information is incorrect
  5. Fill out the form with as much detail as possible
  6. Click "Submit Report" to send your feedback

Method 2: From Reviews Tab

  1. Open the facility details by clicking on a facility
  2. Go to the Reviews tab
  3. Look for the "Report an Issue" option below the review form
  4. Click this option to open the reporting form
  5. Select "Incorrect Facility Information" as the issue type
  6. Provide details about what information is incorrect
  7. Click "Submit Report"

Method 3: Contact Support

  1. Go to the Contact page by clicking "Contact Us" in the footer
  2. Fill out the contact form
  3. Select "Report Incorrect Information" as the subject
  4. In the message field, include:
    • The name of the facility
    • The incorrect information you've noticed
    • The correct information (if you know it)
    • Any supporting evidence or sources (if available)
  5. Click "Send Message" to submit your report

What to Include in Your Report

To help us process your report quickly and accurately, please include:

  • Facility name exactly as it appears in FindHosi
  • Specific information that is incorrect
  • Correct information to replace it (if known)
  • Source of the correct information (e.g., facility website, phone call to facility, personal visit)
  • Date when you verified the correct information
  • Screenshots or photos showing the correct information (if applicable)
  • Your contact information (email) in case we need to follow up

What Happens After You Report

Here's what happens after you submit a report about incorrect facility information:

  1. You'll receive a confirmation that your report has been submitted
  2. Our team will review your report within 1-3 business days
  3. We'll verify the information you've provided against reliable sources
  4. If confirmed, we'll update our database with the correct information
  5. The changes will be visible in the app after the next data refresh
  6. You may receive a follow-up email if we need additional information or to confirm the changes have been made

OpenStreetMap Contributions

FindHosi uses data from OpenStreetMap (OSM), a collaborative mapping project:

  • When you report incorrect information, we not only update our database but also contribute corrections back to OpenStreetMap when appropriate
  • This helps improve the data for everyone, not just FindHosi users
  • If you're familiar with OpenStreetMap, you can also directly edit the OSM data at openstreetmap.org
  • Your contributions to OpenStreetMap will also benefit FindHosi users
